Who we are
Maybush Pharmacy, 99 Rownhams Road, Maybush, Southampton SO16 5EB, is a GPhC-registered community pharmacy providing NHS and private services. We are the data controller for the personal information described in this policy. You can reach us on 023 8077 1211 or in the pharmacy.
What we collect
- Your name, address, postcode, date of birth and contact details.
- Your NHS number, GP surgery and prescription details.
- Health information needed to supply medicines safely, including allergies, current medication and relevant medical history.
- Details you send through the callback form on this website: name, phone number, postcode, the topic you selected and any message you write.
- Records of deliveries, consultations, vaccinations and clinic appointments.
Why we use it
We use your information to dispense and deliver medicines, provide NHS services such as Pharmacy First, New Medicine Service and vaccinations, keep accurate patient records, answer your enquiries and meet our legal and professional obligations.
Our lawful bases under UK GDPR are: performance of a task in the public interest and provision of health care (Articles 6(1)(e) and 9(2)(h)), compliance with legal obligations, and — for optional contact such as marketing or a requested callback — your consent.
Who we share it with
We may share information with your GP surgery, NHS England, the NHS Business Services Authority, hospitals and other healthcare professionals involved in your care, and with our regulated IT and delivery suppliers acting on our instructions. We never sell your data. Information is only shared beyond your care team where the law requires it or you have asked us to.
How long we keep it
Pharmacy records are retained in line with the NHS Records Management Code of Practice — generally eight years for adults, and until a child's 25th birthday for children. Website enquiry details are deleted once your query is resolved, and within 12 months at the latest.
Keeping it safe
Records are stored on secure, access-controlled NHS-approved systems and in locked areas of the pharmacy. Only trained staff who need your information to provide your care can see it, and all staff work under a duty of confidentiality.
Your rights
You can ask for a copy of your information, ask us to correct it, object to or restrict certain uses, withdraw consent where we rely on it, and complain about how we handle it. Some rights are limited where we must keep health records by law.
